This necessary agreement will allow you to\u00a0refer your patients for CBCT scans with appropriate justification.<\/p>\n<p>This simply requires you and your colleagues at your practice to:<\/p>\n<blockquote>\n<ul>\n<li><em>confirm that you refer in accordance with published guidance criteria<\/em><\/li>\n<li><em>confirm that you have fulfilled the standard education requirements in the use of ionising radiation and radiation protection and<\/em><\/li>\n<li><em>provide certification to show that you have undertaken additional training for CBCT Referrers<\/em><\/li>\n<\/ul>\n<\/blockquote>\n<p>Please complete and return your\u00a0agreement along with copies of your relevant CPD certificates as soon as practicable so that you can start to refer patients for scans.<\/p>\n<p><a href=\"https:\/\/hartogdental.co.uk\/dentists-area\/wp-content\/uploads\/sites\/2\/2019\/03\/20190308-hartogcbct-SLA.pdf\">\u00a0<\/a><a href=\"https:\/\/hartogdental.co.uk\/dentists-area\/wp-content\/uploads\/sites\/2\/2019\/03\/20190314-hartogdental-cbct-SLA.pdf\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-3045 size-full\" src=\"https:\/\/hartogdental.co.uk\/dentists-area\/wp-content\/uploads\/sites\/2\/2019\/03\/pdf-download-icon.png\" alt=\"\" width=\"60\" height=\"65\" \/><\/a>Download :\u00a0<a href=\"https:\/\/hartogdental.co.uk\/dentists-area\/wp-content\/uploads\/sites\/2\/2019\/03\/20190314-hartogdental-cbct-SLA.pdf\"><strong>CBCT Service level agreement<\/strong><\/a><\/p>\n<p>Please also complete and return a patient CBCT\u00a0referral form outlining all of your requirements.<\/p>\n<h2>How to refer when you are certified<\/h2>\n<p>Following attendance at one of our CBCT scan referrers lectures or a similar lecture elsewhere, and submission of your service level agreement and certificates we will be delighted to accept your referrals.<\/p>\n<p>Please download and complete a CBCT referral form below in clear block capitals.
